We collaborate with various transportation providers as well as a number of community organizations to improve the health and safety of the communities we serve.

Bay Cities also provides NEMT and ambulance services.

Scheduling a Ride

Contact our call center Monday-Friday between 8 a.m. and 5 p.m. at 1-877-324-8109 to arrange your transportation

Please call at least two business days in advance, if possible. Your ride may not be eligible if we do not have at least two days notice.

We will arrange the best transportation for your needs. This may involve scheduling a volunteer who gives rides to health care appointments.

Mode of Transportation

The kind of vehicle is based on your medical condition at the time of the appointment.

This may mean one of the following:
Bus Tickets/Passes
Car
Taxi
Van
Wheelchair Van
Stretcher Van
We may reimburse driving expenses to you, a friend or family member instead of offering other transportation when medically appropriate.

Securing Transportation Services for Umpqua Health members

Bay Cities Brokerage works with health care providers and facilities to meet the medical transportation needs for Umpqua Health Members.

Call us directly to arrange eligible transportation for Umpqua Health Coordinated Care Organization (CCO) members.

To schedule transportation for a member please call 1-877-324-8109

For those caring for individuals with recurring needs like chemotherapy or dialysis, please call us to set up a standing order and book trips up to 30 days in advance.

Bay Cities Brokerage can also assist with unscheduled, after-hours transportation, such as discharges home from a hospital or other unplanned needs.

How do I ask for a ride?

‘The Call Center is open Monday through Friday, 8am to 5pm to schedule a ride. If you have questions, concerns, an urgent need, or you need to cancel a ride, we are available 24/7/365 to answer your call. Please call at least two (2) business days before your health care appointment so we can make sure you get the ride you need.

Who can call to set up a ride for me?

You, a relative or guardian, caregiver, or someone who works where you live. To protect your privacy, the person will need to know at least three of the following items: your name, date of birth, address, phone number, or OHP ID number.

What if I can’t call two (2) business days before my appointment?

If you are leaving the hospital, going to urgent care, or need to get to or from a chemotherapy or dialysis appointment, call us and we will set up a ride for you.

What do I need when I call?

Please have:

Your home address and phone number
Your mailing address (for bus tickets/passes)
The day and time of your health care appointment
The complete address for your health care appointment
Your caregiver’s name
The general medical reason for your appointment.
We will ask a few other questions to make sure you get the right service.
